RELEASE OF LIABILITY AND INFORMED CONSENT & PHOTOGRAPHY RELEASE FORM

By my signature below, I authorize and consent to the services of One Stop Beauty School (OSBS) students under the supervision of an instructor. I also, hereby grant One Stop Beauty School, hereinafter referred to as “The School,” the absolute and irrevocable right and permission, in respect of the photographs and/or video taken of me alone or with others, to use, re-use, publish, and re-publish the same in whole or in part, individually or in conjunction with other photographs or video, and conjunction with any printed matter, or electronic matter, in any media now or hereafter known, and for any purpose whatsoever; and to use my name in conjunction in addition to that if the school so chooses.

Our students are in training. Please expect the following:

Service time will be LONGER than usual at a professional salon.
Results might not be PERFECT compared to licensed professionals.
The student is NERVOUS and trying their BEST.
Your visit is part of their LEARNING and SUCCESS.

I acknowledge that no guarantee has been made about the results of my services and/or treatments.

I acknowledge I am ok with disinfected tools used on me OR I can purchase a $10 personal kit. I will bring my kit with me to each appointment, or I can purchase a new one.

I further agree to voluntarily release, forever discharge, and agree to indemnify and hold harmless OSBS, its students, educators, agents, and employees from any and all losses, claims, injuries, or damages, sustained or incurred by me .

I am aware and understand that receiving any chemical service can, on some individuals, cause an allergic reaction. I fully understand that this reaction can occur at any time even if I have received this service on previous occasions.

I further agree not to hold my student or OSBS liable for all adverse health reactions from the service(s).

I hereby affirm that I understand that Nails and Skin products contain reactive chemicals which may result in allergic reactions in some Individuals and hereby assume all risk of personal injury which I may sustain because of the application.

I understand OSBS has a NO REFUND policy for fees charged in connection with services rendered. The only exception to this policy is in-school credit, which will be granted upon OSBS approval. No other exceptions to this policy are permitted.

I hereby release and discharge the School from any and all claims and demands arising out of or in connection with the use of the photographs or video, including without limitation any and all claims for libel or invasion of privacy.

This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of the School.

I am of full age (18) and have the right to contract in my own name. If I am not yet 18 years of age, a parent or guardian has authorized my signature. I have read the foregoing and fully understand the contents thereof.

I am aware tipping/giving gratuity to students is the best form of encouragement. Your generosity will be greatly appreciated.
